Project: WFP Operations Building Seismic Retrofit - Rebid Change Order No.001 Change Order Effective Date: 7/10/17 CONTRACTOR By Date: 7o/fes Officer Exhibit A - Description of Changed Work Work Change Directive —No. 003 — Modify Installation of Anchor Rods and Place Grout in Empty Cells for Second Floor Precast Floor Panels: WCD —No. 003 is a force account revision for $8,000.00 and the addition of 10-working days. Justification: - The existing record drawings show the hollow core cells in the second floor precast floor panels to be grouted. During construction it was found that the majority of the cells were only partially grouted or not grouted at all. Due to the lack of grouting additional investigation of the cells was necessary. Anchor rod installation was modified by adding screens, epoxy and anchor rods. Grouting of the empty cells was necessary. Payment is for the additional work required to investigate existing conditions and modify the installation of the seismic strengthening on the second floor. Work Change Directive—No. 004—Mechanical Chase Revisions: WCD—No. 004 was agreed to in the lump sum amount of$8,761.00 and the addition of 3 -Working Days. Justification: - Seismic strengthening was required in a mechanical chase on the second floor. Second floor demolition plan does not show demolition in the mechanical chase. Second floor seismic strengthening plan does not show installation of strengthening in the mechanical chase. Payment is for the additional work needed to demo and restore the mechanical chase. Work Change Directive —No. 005 —Remove Additional Roofing Above Room 216: WCD—No. 005 was agreed to in the lump sum amount of$361.15 and 0-Working Days. Justification: - Prior to the plant expansion in 1993, room 216 was an exterior room and was roofed. After the 1993 expansion room 216 became an interior room.
