Project: WFP Operations Building Seismic Retrofit-Rebid <br /> Change Order No.002 <br /> Change Order Effective Date: 9/15/17 <br /> CONTRACTOR <br /> By ./ Date: fig7/ <br /> Officer <br /> Exhibit A - Description of Changed Work <br /> Work Change Directive—No. 001.1 Flooring Revisions on Second Floor: WCD- <br /> 001.1 is a lump sum amount revision of($2032.00). <br /> Justification: - The contract plans called for the removal and replacement of existing <br /> flooring in the operators' offices and lunchroom. The WFP is beginning a remodel of <br /> the operators' offices and will be replacing the existing carpeting as part of that contract. <br /> The existing vinyl flooring in the kitchen is approximately 10-years old and in good <br /> condition. There is no need to remove and replace it. The credit of$2,032.00 represents <br /> the difference between partial removal and complete removal, the protecting and <br /> cleaning of the existing. The revision also accounts for the additional work to revise the <br /> tile replacement over the seismic strengthening in the restrooms. <br /> Work Change Directive—No. 002 — Control-Panel Room Revision: WCD-002 is a <br /> lump sum revision for $2,818.00. <br /> Justification: The panel room and control room in the Operations Building use to be <br /> two separate rooms. Recent PLC upgrade work made existing electrical panels obsolete. <br /> The panels and the wall between the control room and panel room were removed to <br /> enlarge the control room. The panels room and the control room are now one room. To <br /> eliminate the abrupt edge where the wall between the rooms use to be, the flooring was <br /> extended to cover the entire floor. This work change directive covers the cost for the <br /> grouting in floor penetrations, additional floor leveler and additional linoleum. <br /> Work Change Directive—No. 011 — FRP Under Rollup Door: WCD—No. 011 is a <br /> lump sum revision for $6,494.00 and the addition of 1-working day. <br /> Justification: - In room 216, the contract plans called for an 8x6x1/2 angle to be <br /> installed along the east wall of the room.
