Project: WFP Operations Building Seismic Retrofit -Rebid <br /> Change Order No.002 <br /> Change Order Effective Date: 9/15/17 <br /> a man door. The angle would have created a trip hazard at the doors. Additionally, <br /> because of the doors, there was no wall to attach the vertical leg of the angle. An FRP <br /> strip was designed and installed in lieu of the angle to avoid a tripping hazard and to <br /> mitigate the missing wall. <br /> Work Change Directive—No. 012 —Additional Labor to Investigate Non-Grouted <br /> Cells: WCD—No. 012 was agreed to in the lump sum amount of$744.00 and the <br /> addition of 1 -Working Day. <br /> Justification: - The design of the seismic strengthening required a minimum spacing of <br /> 4-feet for grouted cells and an average grout depth of 8-inches in the precast floor <br /> panels for the second floor of the Operations Building. After it was discovered that <br /> some cells were missing grout or only partially grouted, additional investigation of the <br /> cell grouting was required. The contactor provided equipment and labor to drill into the <br /> floor panels so the City could confirm the presence and depth of grout in the cells. <br /> Work Change Directive—No. 013 — Provide 4-inch Seismic Joints In Lieu of 3- <br /> Inch: WCD-013 was agreed to in the lump sum amount of$2289.00 and the addition of <br /> 1-working day. <br /> Justification: - The contract plans called for installing 3-inch diameter flexible seismic <br /> joints on the four vent pipes for the alum and polymer storage tanks. During <br /> construction, the vent piping was found to be 4-inches in diameter. WCD-013 provided <br /> a material only credit for the 3-inch diameter seismic joints and paid for the material <br /> only costs of the 4-inch diameter seismic joints. <br /> Work Change Directive—No. 014 Hollow Core Panel Grouting: WCD-014 is a lump <br /> sum revision of$7,340.87 and the addition of 10-working days. <br /> Justification: The contract drawings and recorded drawings, for the Operations <br /> Building, show the cells in hollow core floor panels to be grouted. It was discovered <br /> during construction, that the majority of the cells were either partially grouted or not <br /> grouted at all. The design of the seismic reinforcing was revised to account for the <br /> partially grouted or non-grouted cells. The anchor bolt installation was modified and <br /> empty cells in the floor panels were grouted. Work change directive covers the <br /> additional costs to modify the anchor bolts and grout the empty cells. Work change <br /> directive 014 is an extension of work change directive 003.
