1 <br /> (7) All assumed names or aliases which have been or are used by any <br /> 2 person whose name appears on the application; and <br /> 3 (8) Such other information as the licensing authority may reasonably <br /> require in connection with the application. <br /> 4 <br /> 5 <br /> Section 11 : Fees and Costs. <br /> 6 11 .1 An annual fee shall be paid in full at the time of application as <br /> 7 set out in this Section. An applicant who proposes to conduct activities at <br /> 8 more than one location shall pay a separate fee for each location for which <br /> 9 a license may be issued. A fee is not refundable and after July 31 , <br /> 10 one-half of the annual fee set forth in Section 11 .2 shall be charged. <br /> 11 11 .2 Annual fees and renewal fees for the following license is <br /> 12 established as follows: <br /> 13 <br /> Tobacco Vending Machine License Fee: $25/year <br /> 4 per tobacco <br /> vending machine <br /> 5i <br /> Renewal Fee: $25/year <br /> 16 per tobacco <br /> vending machine <br /> 17 <br /> Retailer Tobacco Product License Fee: $50/year <br /> 18 <br /> Renewal Fee: $50/year <br /> 19 <br /> 20 11 .3 Retailers may purchase signs as required by Section 5 at the <br /> 21 office of the licensing authority. <br /> 22 Section 12: License - Term, Renewal , Transferability. <br /> 13 12.1 A license issued under this chapter shall expire on December 31st <br /> 24 of each year. Such licenses may be renewed by the licensing authority if, <br /> 25 upon application, it appears that the applicant continues to meet the <br /> 26 requirements of this chapter. Such licenses may be renewed by the licensee <br /> -'7 for the succeeding year by making application therefor with the City Clerk <br /> 8 at least ten (10) days prior to the expiration date. <br /> l <br /> 7 <br />
