Exhibit A
Continued
3.1 Geotechnical Data Report
CCI will provide a geotechnical data report summarizing the subsurface conditions encountered
during site reconnaissance and explorations. The data report will include Summary Boring Logs and
subsurface interpretation of the existing dike and foundation conditions. Cornforth will provide the
geotechnical data report to be included into the bid documents for contractor's review.
3.2 Dike Repairs Final Design
Cornforth will provide input for the final geotechnical design of the dike repairs. Analyses will be
updated based on the final dike repair design. Final design will include input on drawings for the
90% and 100%design submittals for review and comment by DD5.
Design recommendations will be provided with the intent of addressing dike deficiencies and
achieving an acceptable rating from the USACE, and with the understanding that geotechnical
information is needed for dike design purposes, but not a requirement of eligibility for the PL84-99
program. The geotechnical dike evaluation and analysis will be performed in general accordance with
the following USACE publications:
• EM 500-1-1 "Civil Emergency Management Program" PL84-99 requirements
• EM 1110-2-1913 "Design and Construction of Levees"
• EM 1110-2-1902 "Slope Stability"
• ER 1110-2-1806 "Earthquake Design and Evaluation for Civil Works Projects"
• ETL 1110-2-569 "Design Guidance for Levee Underseepage"
• EC 1110-2-6067 "Engineering and Design: USACE Process for the National Flood Insurance
Program (NFIP) Levee System Evaluation"
• ETL 1110-2-6-571 "Guidelines for Landscape Planting and Vegetation Management at Levees,
Floodwalls,Embankment Dams, and Appurtenant Structures"
Task 4 — Environmental Studies to Support Regulatory Compliance
During Phase 1 of the project, the Otak team completed the Wetland and Stream Delineation Report to
document baseline environmental conditions in the project area. Environmental data was collected
to inform proposed project alternatives analyses during Phase 1, and will be used to support the City
on the permit applications and to demonstrate regulatory compliance required for Phase 2 of the
project for the preferred design of the dike repairs. The preferred design is only located with the City
of Everett limits west of the I-5 bridge, and as such, permits from Snohomish County will not be
required.
The anticipated permits and required compliance applicable to this project include:
• U.S. Army Corps of Engineers (Army Corps) Section 404 permit
• U.S. Army Corps Section 10 permit
DD5 Dike Repairs and Improvements Project 4
otak
k:\project\32700\32758\Contract\Phase2_Scope\Scope for Phase 2-DD5 dike repairs final_17_1010.docx
