12 <br /> AMENDMENT NO. 6 TO PROFESSIONAL SERVICES AGREEMENT BETWEEN <br /> THE CITY OF EVERETT <br /> AND <br /> OTAK,INC. <br /> The City and Otak, Inc. agree to amend and modify their professional services agreement dated <br /> September 16,2009 (the"Agreement") as follows: <br /> 1. The effective date of this Amendment is (month) , (day) , (year) <br /> 2. Section 3 of the Agreement is modified to read as follows: <br /> Time of Beginning and Completion of Performance: This Agreement shall commence as <br /> of the date of execution of this Agreement and shall be completed by December 31,2019. <br /> 3. Regardless of the date(s) on which this Amendment No. 6 is signed by the parties, the parties <br /> agree that the Agreement does not expire on January 1, 2018. The parties agree that the <br /> Agreement is continuously in effect since September 16,2009. <br /> 4. All terms, conditions and provisions of the Agreement remain in full force and effect except as <br /> expressly modified by this Amendment. <br /> 5.
