• <br /> 1 Such insurance as required herein shall be maintained in full force and <br /> 2 effect for the full period to be covered by the license applied for, and <br /> 3 failure to do so shall result in automatic suspension. <br /> 4 Applicant shall submit a Certificate of Insurance to the City at the <br /> 5 time of license application indicating compliance with the insurance <br /> 6 requirements set forth herein and naming the City of Everett as an <br /> 7 additional insured on the insurance coverage. <br /> 8 Section 11 : FAILURE TO PAY JUDGMENT <br /> 9 Failure to pay any judgment arising out of the limousine business <br /> 10 within thirty (30) days after the judgment becomes final or to make any <br /> 11 payment pursuant to such judgment shall result in denial , suspension, or <br /> 12 revocation of a for-hire license. No limousine license shall be renewed if <br /> 13 payments on an outstanding final judgment are delinquent. <br /> 14 Section 12: LIMOUSINE DRIVER'S LICENSE - REQUIRED <br /> 15 <br /> 16 A. It is unlawful for any person engaged in the business of operating <br /> limousines to employ as the driver of any such vehicle, or permit any <br /> 17 such vehicle to be operated by a driver who does not have in his/her <br /> possession a valid limousine driver's license. <br /> 18 B. It is unlawful for any person to drive or operate a limousine for-hire <br /> 19 in the City of Everett unless there is a valid limousine business <br /> license issued by the City covering said vehicle. <br /> 20 C. It is unlawful for any person to drive or operate a limousine for-hire <br /> 21 in the City of Everett without first having obtained a valid license to <br /> do so, which license shall be known as a limousine driver's license. <br /> 22 The license shall be obtained in the following manner: the applicant <br /> shall file an application on a form furnished by the City Clerk which <br /> 23 shall be signed and sworn to by the applicant and shall contain: the <br /> applicant's full name, place of residence at the time of application <br /> 24 and for a period of two years prior thereto, age, race, height, color <br /> of eyes and hair, place of birth, whether a citizen or noncitizen, <br /> 25 last place of employment, whether she/he has been convicted of a <br /> felony, misdemeanor or bail forfeiture related to the ability and <br /> 26 fitness of the applicant to engage in the operation of a limousine, <br /> whether she/he has been previously licensed as a driver or chauffeur <br /> 27 and if so, when and where, whether such license has ever been suspended <br /> or revoked and for what cause and such other information the City Clerk <br /> 28 may require which she/he deems reasonably necessary to aid in the <br /> enforcement of this ordinance. Said application shall be filed with <br /> 29 the City as a permanent record and shall contain the names of four <br /> persons, not relatives, who have known the applicant for at least two <br /> 30 years past. <br /> 31 Section 13: DRIVER'S LICENSE - QUALIFICATIONS <br /> 32 A.
