• <br /> • <br /> 1 1 . Must be at least eighteen years of age; <br /> 2 2. Must possess a valid state of Washington motor vehicle operator's <br /> license; <br /> 3 <br /> 3. Be able to read, speak, and write the English language; <br /> 4 <br /> 4. Must be free from any infirmity of the body or mind which would <br /> 5 render the applicant unfit for safe operation of a motor vehicle, <br /> must not be found to be addicted to the use of intoxicating <br /> 6 liquor, dangerous drugs or narcotics within two years of the date <br /> of application, and shall have submitted to a medical examination <br /> 7 at the applicant' s expense by a licensed physician and filed with <br /> the City Clerk a certificate from said physician certifying his <br /> 8 fitness as a for-hire driver. Such examination shall be required <br /> at least every four (4) years but not for renewals of such <br /> 9 license; provided however, the City Clerk may at any time at <br /> her/his discretion require any licensee to be so examined and to <br /> 10 secure such a certificate or renewal thereof. Any applicant who <br /> fails to pass a satisfactory medical examination may be <br /> 11 re-examined after thirty days from the date of original <br /> examination, and if following such re-examination the certificate <br /> 12 herein required is issued, the applicant may be licensed in <br /> accordance with the provisions of this ordinance provided all <br /> 13 other requirements are met; <br /> 14 5. Not have been convicted or forfeited bail to any of the following <br /> when it involves crimes pertaining to controlled substances, <br /> 15 alcohol , prostitution, gambling, physical violence, or crimes <br /> directly related to the applicant's honesty and integrity (such <br /> 16 as, but not limited to, fraud, larceny, extortion) or is directly <br /> related to the ability and fitness of the applicant to engage in <br /> 17 the operation of a limousine for hire: <br /> 18 a. Felony within ten years preceding date of application; <br /> 19 b. Gross misdemeanor or misdemeanor within two years preceding <br /> date of application; <br /> 20 <br /> 21 c. Criminal traffic within one year preceding date of <br /> application; <br /> 22 d. Traffic infraction within six months preceding date of <br /> application; <br /> 23 <br /> 24 6. Show proof that he is covered by an insurance policy which meets <br /> the minimum requirements as set forth in this ordinance; and <br /> 25 <br /> 26 Section 14: FINGERPRINTS AND PHOTOGRAPHS OF APPLICANTS <br /> 27 Every applicant for a limousine driver's license must submit, at the <br /> 28 applicant's expense, to fingerprinting by the Everett Police Department and <br /> 29 shall submit with his application, in duplicate, a full face photograph of <br /> 30 the applicant; said photographs to be of the size of one inch square.
