I <br /> 2.2 PROPOSED PRODUCT LIST <br /> A. Within 14 days after date of Notice to Proceed,prepare and submit to the Engineer list of major <br /> products proposed for use,with name of manufacturer,supplier,trade name,subcontractor and <br /> model number of each product. Provide a schedule of specific target dates for the submission <br /> and return of shop drawings required by the contract documents. <br /> B. For products specified only by reference standards, indicate manufacturer, trade name, model <br /> or catalog designation, and reference standards. <br /> C. The list and schedule shall be updated and re-submitted when requested by the Engineer. <br /> 2.3 BREAKDOWN OF CONTRACT PRICE <br /> A. At or before the preconstruction meeting, submit a complete breakdown of all lump sum bid <br /> items showing the value assigned to each part of the work, including an allowance for profit <br /> and overhead adding up to the total lump sum contract price. <br /> B. Breakdown of lump sum bids shall be coordinated with the items in the schedule and shall be <br /> in sufficient detail to serve as the basis for progress payments during construction. <br /> C. Engineer will review the contract price breakdown and may request items to be further broken <br /> down or for more items be added in order to facilitate tracking of work progress for payment. <br /> D. Preparatory work, bonds, and insurance required in setting up the job will be allowed as a <br /> separate entry on the cost breakdown but shall not exceed 5 percent of the total base bid. I <br /> E. Upon acceptance of the breakdown of the contract price by the Engineer,it shall be used as the <br /> basis for all requests for payment. I <br /> 2.4 PUBLIC SAFETY AND CONVENIENCE <br /> A. Project Health and Safety Plan: <br /> 1. Develop, publish, and implement an overall Project Health and Safety Program for the <br /> Project. This Program shall conform to all applicable codes. The written Safety Program <br /> shall be provided within thirty(30)days after the receipt of the written Notice to Proceed. <br /> The Plan shall be assembled to address project specific health and safety issues to both the <br /> public and on-site personnel. The plan shall include the following items when they apply: I <br /> • Employee orientation • Excavations <br /> • Safety inspections • Sanitation <br /> • Instruction and training • Chlorine safety <br /> • Accident reporting • Hazardous materials <br /> • Signs and barricades • Hazardous communications <br /> • Fire prevention and protection program <br /> • Welding, cutting and burning • Job hazard analysis <br /> • Painting and surface treatment • First aid/medical facilities <br /> • Electricity • Personal protective equipment <br /> • Machinery and mechanized • Confined space entry plan <br /> equipment • Shoring plan <br /> Legion Golf Course Stormwater Detention Project Submittal Procedures <br /> WO#UP3620 01 33 00 - 8 <br />
