<br /> CITY OF EVERETT SPECIAL PROVISIONS
<br /> Measurement for Cement Concrete Pavement, Type III (H.E.S.) shall be per square yard as
<br /> delineated by the Contractor and approved by the Inspector prior to removal.
<br /> The unit price per square yard for Cement Concrete Pavement Base, Type III (H.E.S), shall
<br /> be full compensation for all materials, tools, labor and equipment necessary to complete
<br /> the Work in accordance with the Plans, Section 5 of the Standard Specifications and 5-
<br /> 1 06.3(2) and 5-06.3(3) of these Special Provisions.
<br /> Bid Item 33 - HMA, CL 1/2" PG 64-22 for Permanent Trench Patch
<br /> Measurement and Payment: Unit Price per Ton (TON)
<br /> Measurement for HMA, CL 1/2" PG 64-22 shall be by the ton, recorded on certified weight
<br /> tickets in accordance with 1-09.2 WEIGHING EQUIPMENT for HMA placed within the limits
<br /> of dimensions defined in the Work or shown on the Plans, Standard Specifications, Section
<br /> 5-06 of these Special Provisions, or as otherwise approved by the Engineer. The
<br /> measurement and payment limits for measuring the width of this bid item shall not exceed
<br /> the saw-cut limits shown on the Drawings. No deduction will be made for the weight of
<br /> asphalt binder, blending sand, mineral filler, or other component of the mixture.
<br /> The unit price for this bid item shall include all labor, materials, equipment and related work
<br /> necessary to furnish, machine-place, compact, roll and perform density tests on multiple
<br /> lifts of HMA — CL 1/2" PG 64-22 pavement including streets, alleys, street crossings, and
<br /> driveway aprons as shown on the Plans.
<br /> Miscellaneous HMA used for pavement repair, thickened edges, extruded and wedge
<br /> curbing, placing and compacting pre-leveling, leveling and wearing courses, feathering
<br /> pavement is included in this bid item with no direct compensation made.
<br /> Furnishing and applying tack coat, prime coat, joint seal and crack sealing asphalt is to be
<br /> included in this bid item with no direct compensation made. All costs for "Anti Stripping
<br /> Additive" and "Compaction Adjustment" shall be included in the unit contract price per ton of
<br /> the HMA with no direct compensation made.
<br /> The unit price for this bid item shall include all labor, materials, equipment and related work
<br /> necessary to furnish, place, compact, roll and perform density tests on ATB placed in
<br /> accordance with the Plans, Standard Specifications, and these Special Provisions.
<br /> Bid Item 34 - Cement Concrete Sidewalk Restoration
<br /> Measurement and Payment: Unit Price per Square Yard (SY)
<br /> Measurement shall be per square yard as delineated by the Contractor and approved by
<br /> the Inspector and adjacent property owner prior to removal.
<br /> The unit price per square yard for restoration of existing concrete sidewalks shall be full
<br /> compensation for final sawcut and all materials, tools, labor and equipment necessary to
<br /> complete the work in accordance with the Plans, COE Standard Drawings, Standard
<br /> Specifications and these Special Provisions.
<br /> R Bid Item 35 - Cement Concrete Curb and Gutter, Type 1
<br /> Measurement and Payment: Unit Price per Linear Foot (LF)
<br /> Measurement shall be per linear foot as delineated by the Contractor and approved by the
<br /> Inspector prior to removal.
<br /> The unit price per linear foot for restoration of concrete curbs or curb and gutter and shall
<br /> be full compensation for all materials, tools, labor and equipment necessary to complete
<br /> the Work in accordance with the Plans, COE Standard Drawings, Standard Specifications
<br /> and these Special Provisions, including sawcutting, removal, loading, hauling and disposal.
