It will <br /> 8 also list estimated quantities, units of measurement, the items of work, and the materials <br /> I 9 <br /> 10 to be furnished at the unit bid prices. The bidder shall complete spaces on the proposal <br /> form that call for, but are not limited to, unit prices; extensions; summations; the total bid <br /> 11 amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment <br /> I 12 of addenda; the bidder's name, address, telephone number, and signature; the bidder's <br /> 13 UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor's <br /> 14 Registration Number; and a Business License Number, if applicable. Bids shall be <br /> 15 completed by typing or shall be printed in ink by hand, preferably in black ink. The <br /> I16 required certifications are included as part of the Proposal Form. <br /> 17 <br /> 18 The Contracting Agency reserves the right to arrange the proposal forms with alternates <br /> I 19 and additives, if such be to the advantage of the Contracting Agency. The bidder shall <br /> 20 bid on all alternates and additives set forth in the Proposal Form unless otherwise <br /> 21 specified. <br /> I 22 <br /> 23 1-02.6.GR1 <br /> 24 Preparation of Proposal <br /> I25 <br /> 26 1-02.6.OPT10.GR1 <br /> 27 (June 1, 2017) <br /> I 28 The Bidder shall submit with the Bid a completed Underutilized Disadvantaged <br /> 29 Business Enterprise (UDBE) Utilization Certification, when required by the Special <br /> 30 Provisions. For each and every UDBE firm listed on the Bidder's completed <br /> 31 Underutilized Disadvantaged Business Enterprise Utilization Certification, the Bidder <br /> 32 shall submit written confirmation from that UDBE firm that the UDBE is in agreement <br /> 33 with the UDBE participation commitment that the Bidder has made in the Bidder's <br /> 34 completed Underutilized Disadvantaged Business Enterprise Utilization Certification. <br /> 1 35 WSDOT Form 422 031U (Underutilized Disadvantaged Business Enterprise Written <br /> 36 Confirmation Document) is to be used for this purpose. Bidder must submit good faith <br /> 37 effort documentation only in the event the bidder's efforts to solicit sufficient UDBE <br /> I 38 participation have been unsuccessful.
