Upon suspension, revocation or termination of any other <br /> 2 license/certificate issued by the Secretary, such person shall cease to drive or attend an <br /> 3 ambulance and/or emergency first aid vehicle and no person shall employ or permit such <br /> 4 individual to drive or attend an ambulance and/or emergency first aid vehicle. <br /> 5 <br /> 6 Section 10: Reports. <br /> 7 A. Each licensee of an ambulance and/or emergency first aid vehicle <br /> 8 hereunder shall maintain accurate records, upon such forms as may be provided or <br /> 9 prescribed by, and containing such information as may be required by, the License <br /> 10 Officer, concerning the transportation of each patient within the City of Everett or <br /> 11 from one place herein to another place within or beyond its limits. Such records shall <br /> 12 be available for inspection by the License Officer at any reasonable time. <br /> 13 B. The provisions of Subsection (A) shall apply with equal force in case the <br /> 14 patient shall die before being so transported in such ambulance and/or emergency first <br /> 15 aid vehicle or dies while being transported therein, or at any time prior to the admission <br /> 16 of the patient to a hospital. <br /> 17 <br /> 18 Section 11: City Emergency Medical Services Director. <br /> 19 A. Mayor shall appoint a licensed physician to serve as an advisor to the <br /> 20 City's emergency medical services program as provided by the Everett Fire <br /> 21 Department. <br /> 22 B. Such person shall have expertise in the area of emergency medical <br /> 23 services. <br /> 24 C. Such person shall provide medical recommendations to the Everett <br /> 25 Emergency Services Advisory Committee and shall act as an ex-officio non-voting <br /> 26 member of the Committee as set forth in Section 6 of this ordinance. <br /> 27 D. Such further responsibilities as are deemed necessary shall be set forth in <br /> 28 a personal service contract. <br /> 29 <br /> 30 <br /> 31 <br /> 32 -9- <br />
