No manufacturer name and model number was submitted for the soft serve machine, Item#12, the <br /> heat treaded machine, Item #13, food preparation sink, Item #15, and the freezer, Item #17. The <br /> manufacturer names and model numbers for this equipment must be submitted. <br /> 11. Food service establishments wanting to continue operation during a remodel must submit written <br /> documenlation stating the procedures that wili be used to ensure food safety during the remodel. <br /> The documentation must include the following: <br /> a) Where and how hancwash facilities will be set up <br /> b) What methods will be used for maintaining proper temperatures of all potentially hazardous <br /> foods <br /> c) Where produce will be prepared " <br /> d) Where dishwashing will occur <br /> e) What procedures will be followed if interruptions in water, power or sewage disposal occur <br /> fl What type of dust barriers will be used and where will they be used <br /> g) A floor plan of any temporary'ood preparation areas <br /> 12. Submit a revised floor plan, drawn to scale, showing location of all equipment, plumbing fixtures <br /> and the like that includes the required additional sinks, requirements and information. The scale of <br /> the drawing must be no smailer than Y< inch equals 1 foot. Please note that plans may be submitted <br /> in an unsecured PDF format. Cut sheets and all other information may also be submiried in a PDF <br /> formal. <br /> Please note that prior to approval to reopen the new facility and use of all remodeled areas, after <br /> the Health District plan review process is completed and remodel construction is finished, a <br /> preoperational inspection must be conducted. <br /> Please contact me if you have any questions.
