I <br /> 1.16 PERFORMANCE ASSURANCE <br /> A. Accepted Bidder: Provide separate 100% performance and 100% payment bonds as described in <br /> Document 00 72 13 -General Conditions. <br /> 1.17 BID FORM REQUIREMENTS 1 <br /> A. Complete requested information in City provided Bid Form found in the Contract Documents. <br /> B. Legibly fill in all blank spaces using non-erasable medium. <br /> C. In the event that the product of a unit price and an estimated quantity does not equal the extended amount <br /> quoted, the unit price shall govern, and the correct product of the unit price and the estimated quantity <br /> shall be deemed to be the amount bid. <br /> D. If the sum of two or more items in a bidding schedule does not equal the total amount quoted, the <br /> individual item amounts shall govern and the correct total shall be deemed to be the amount bid. <br /> E. Bid prices shall include everything necessary for the completion of the Work including, but not limited <br /> to, providing the materials, equipment, tools, plant and other facilities, and the management, <br /> superintendence, labor, and all necessary testing services. <br /> F. No oral,telegraphic, or telephonic bids or modifications will be considered. I <br /> 1.18 BID FORM SIGNATURE <br /> A. Sign Bid Form as follows: <br /> 1. Sole Proprietorship: <br /> a. Signature of sole proprietor. <br /> b. Include words "Sole Proprietor" under signature. <br /> c. Include individual's name and post office address. <br /> 2. Partnership: <br /> a. Signature of at least one of the general partners. <br /> b. Include word "Partner"under signature. <br /> c. Include post office address for firm or partnership. <br /> 3. Corporation: <br /> a. Signature of at least one duly authorized signing officer. <br /> b. Include officer's capacity/title under each signature. <br /> c.
