I <br /> I 5. Disadvantaged/Women's Business Enterprise: To be filled in and signed by the <br /> bidder. <br /> 6. Bidder's Information Statement: To be filled in and signed by the bidder. <br /> 1 7. Non-Collusion Affidavit: To be signed and submitted with the bid. Failure to complete <br /> the aforementioned forms and to submit said forms with the bid shall be due cause for <br /> irejection of bid. <br /> The following forms are to be executed after the award of Contract: <br /> I 1. Contract: This Contract to be executed by the successful bidder in duplicate within ten <br /> (10)calendar days after the award date. <br /> I 2. Payment and Performance Bond: This form is to be executed by the successful bidder <br /> and his surety company in triplicate. The amount of these bonds shall be one hundred <br /> percent(100%)of the amount of the bid and shall be submitted with the contract. <br /> 1 3. Proof of Insurance: Insurance certificates and endorsements shall be obtained and <br /> maintained in force in accordance with Section 00710, Part 15 "Liability and Insurance", <br /> of the Specifications. <br /> I4. Power of Attorney: Attorneys-in-fact who sign bid bonds or contract bonds must file <br /> with each bond a certified and effectively dated copy of their Power of Attorney. <br /> 5. Statement of Intent to Pay Prevailing Wage (L&I Form 700-29) and Affidavit of <br /> Wages Paid (K-700-007-000) from the Contractor, Subcontractor and any agent to the <br /> Subcontractor shall be submitted to the Employment Standards Division, State <br /> I <br /> Department of Labor and Industries, Olympia, Washington. <br /> 6. Monthly Employment Utilization Report (Form 257) completed each month by the <br /> IContractor and Subcontractor($100,000 or more). <br /> 7. Weekly Statement with Respect to Payment of Wages (Form WH347): Contractors, <br /> II Subcontractors, and agents to Subcontractors using Payroll Form WH347) may use State <br /> of Compliance found on back of form. <br /> 8. Weekly Statement of Compliance (Form WH 348). Contractors, Subcontractors, or <br /> I agents to Subcontractors not using Payroll Form WH347 shall attach the Statement of <br /> Compliance Form WH348 to each payroll. <br /> I 9. Approval of Subcontractors: Contractors shall request approval of Subcontractors prior <br /> to their working on the site and certify on a form provided by the City that a written <br /> agreement between the Contractor and the Subcontractor, or between the Subcontractor <br /> Rand any agent to the Subcontractor has been executed. <br /> 10. Construction Progress Schedule: To be submitted as required by Section 00710, Part 5 <br /> "Progress and Completion", of the Specifications. by the Contractor within ten(10) days, <br /> Ior such time as determined by the Engineer, after the date of receipt of notice to proceed. <br /> 11. Traffic Control Plan: Standard Traffic Control Plans are included in the City's Design <br /> I and Construction Standards.
