Project-Water Main Replacement Q Evergreen Way Rebid Change Order No. 001 Change Order Effective Date: March 1, 2019 Added Bid Items New Item 101 —Full Lane Pavement Grind. Add new item# 101 entitled, "Full Lane Pavement Grind" at the agreed to lump sum price of$26,740.00, resulting in a$26,740.00 increase to the Contract price. New Item 102—Adjust Valve Boxes. Add new item 102 entitled"Adjust Valve Boxes", to increase contract value at agreed unit price of$1,230.00 per EA,by adding 20 EA, resulting in a$24,600.00 increase to the contract. New Item 103 Adjust Catch Basins. Add new item 103 entitled"Adjust Catch Basins",to increase contract value at agreed unit price of$1,695.00 per EA,by adding 5 EA,resulting in a$8,475.00 increase to the contract. Washington State Sales Tax An increase to the contract in the amount of$16,105.32 is included for the estimated sales tax payment. JUSTIFICATION Bid Items; 4 Traffic Control Labor, 14 Gravel Borrow,27 Water Service Line 1", 33 Permanent HMA Patch and 38 Vehicle Detection Loops-These items over-ran the estimated plan quantity which requires addition funding. Bid Items; 13 Foundation Material, 32 Cement Concrete Base and, and 38 Permanent Pavement Markings—There was no work associated with these items therefore the quantity is being reduced to zero. New Items; 101 Full Lane Pavement Grinding, 102 Adjust Valve Boxes and 103 Adjust Catch Basins. These are added as compensation for expanding pavement restoration from trench patch width to full lane width. Washington State Sales Tax All changed work represented in subject to WAC 458-20-170 which requires the contractor to collect sales tax.
