• (4) Litter patient may be transported if he meets requirements specified in subsections (2) and (3) . Section 4 Requirement In Addition To General Provisions : In addition to the general requirements , there is the additional re- quirement that the applicant(s) or manager (s) or owner (s) of the applicant have not been convicted of a crime of moral turpitude or intent to defraud within ten (10) years from the date of the application, and furthermore, has not within ten (10) years from the date of application, been released from a penal institution or from active super- vision on parole as a result of the conviction of any of the aforementioned crimes. Section 5 Application Requirements Additional To General Provisions : In addition to the information required to be provided pur- suant to the general requirements for all licenses , applicants for "for-hire vehicles business" license shall furnish the following information: (A) The financial status of the applicant, including the amounts of all unpaid judgments against the applicant and the nature of the transaction or acts giving rise to said judgments : (B) The experience of the applicant in the trans- portation of passengers; (C) Any facts which the applicant believes to him to provide that public convenience and necessity require the granting of a license: (D) The number of vehicles to be operated or con- trolled by the applicant and the location of proposed depots and terminals; (E) For each for-hire vehicle, the company vehicle number therefor, the make, model and identifying color scheme , monogram or insignia, and serial number of the vehicle; (F) If the applicant is a corporation, it shall accompany the application with a list of the names and ad- dresses of all officers , directors and stockholders ; (G) Such further information as the City Clerk may require. Section 6 License-Fee: The license fees are fixed in the amounts shown in the following schedule, and shall be payable annually in advance: For hire vehicles : $50 . 00 Driver' s Permit: 25 . 00 per driver Section 7 License - Issuance: If the City Clerk finds that further for-hire service in the City of Everet is required by the public convenience and necessity, be it taxicab or convalescent coach, as the case may be, and that the applicant is fit, will- ing and able to perform such public transportation and to con- form to the provisions of this chapter, the City Clerk shall is- sue a license stating the name and address of the applicant,
