1 <br /> Ted de Budden N I I [ 2 ' 005 <br /> From: Ted de Budden <br /> Sent: Friday,July 01, 2016 11:57 AM <br /> To: Donald Cunningham <br /> Subject: RE: 2901 Everett Ave <br /> Don, <br /> Thank you for sending me the summary of your observations for the inspection we conducted yesterday at 2901 Everett <br /> Ave. <br /> Here is a summary of my observations based on inspection and research of records that I have conducted. <br /> This overview is not intended to be a complete list of deficiencies within the buildings on this property. <br /> 1. The main building was issued a certificate of occupancy for the conversion of a single family residence to a boarding <br /> house in 2006. It is important to note that records indicate the habitable space that was approved under the permits <br /> issued for the conversion to boarding house was limited to the main and second floors of the building. <br /> 2. The building has been altered to now include habitable use on the basement level and the "loft" levels of the <br /> structure. Rooms have been altered and created that are not consistent with the plans that were approved for the <br /> boarding house conversion. <br /> In addition to the loft and basement occupancy and according to permit records, sleeping rooms#4, 8, 10, 11, 12, 13, <br /> and half bathroom#3 were created and altered beyond the scope of the original boarding house permit. (#C0505-001) <br /> Required permits and approvals for these alterations and expanded uses cannot be located. <br /> 3. Eighteen sleeping rooms have been created within the building.Two rooms created for sleeping use in the basement <br /> lack the minimum size to comply with IPMC code requirements. Room#16 is too small. Room#18 is too narrow.These <br /> rooms should not be occupied. Room#9 is currently being used for sleeping. Room#9 lacks a required sprinkler and <br /> has not been approved for sleeping use,this room should not be occupied. <br /> All rooms in the loft area have not received required approvals for use and should not be occupied. <br /> 4. All stairways in the structure are required to have a minimum of one handrail, some stairs were observed to be <br /> lacking required rails.The stairs leading to the basement has a severe head clearance issue,the upstairs door swings in <br /> the wrong direction over a step and the lower door provides no required landing.All code violations. <br /> 5. Open electrical boxes and connections were visible in the basement area and several outlet covers are missing. <br /> Damaged electrical conductors were observed outside the main building.These conductors appear to serve the <br /> detached garage. <br /> Electrical panels are located in some sleeping rooms and are not readily accessible as required. <br /> 6. Functional smoke alarms were missing in numerous locations throughout the building. <br /> 7. Required fire alarm strobes and annunciators are not provided in some locations within the building. <br /> 8. A detached garage has been converted to habitable space without required permits and approvals. <br /> 9. The required number of parking spaces for this use does not appear to be provided. <br /> In summary,this building has numerous code violations. <br /> This building has been altered and occupied contrary to law and beyond the scope of the original permits. <br /> This is an Unlawful Structure as defined by Code.
