I <br /> Section 00 21 13 <br /> INSTRUCTIONS TO BIDDERS <br /> the Washington State Department of Revenue,whether the amount owed relates to this contract or not. <br /> The amount so deducted will be paid into the proper State fund. <br /> 1.19 Limitations Regarding Contractor's Claims for Damages <br /> Bidders should review the Contract Documents regarding limitations on claims for damages. , <br /> 1.20 Delays and Interference <br /> Bidders should review the Contract Documents regarding delays and interference. 1 <br /> 1.21 Bidder's Check List <br /> A. The Bidder's attention is especially called to the followingforms that must be executed in full as <br /> p Y <br /> required and submitted with the Bid: <br /> 1. Bid Form: The lump sum and unit price items must be shown in the space provided. Show unit <br /> prices in both words and figures. <br /> 2. Proposed Subcontractors Form: To be filled in by the Bidder. <br /> 3. Bid Bond: This form is to be executed by the Bidder and the surety company unless Bid is <br /> accompanied by a certified or cashier's check. The amount of this bond, which shall be not less <br /> than five percent of the Total Bid Amount bid, may be shown in dollars or on a percentage basis. <br /> 4. Minority Business Enterprise: To be filled in and signed by the Bidder. <br /> 5. Non-Collusion Affidavit: To be signed and submitted with the Bid. Failure to complete the <br /> aforementioned forms and to submit said forms with the Bid shall be due cause for rejection of <br /> Bid. I <br /> 6. Signed Copies of all Addenda. <br /> B. The apparent low Bidder and apparent second low bidder shall submit, to the City Clerk, the <br /> following no later than 2 business days after Bid opening, unless the City's project manager in <br /> writing allows additional time: <br /> 1. Bidder's Qualifications Statement: To be filled in and signed by the Bidder. If the Bidder <br /> submitted the Bidder's Qualifications Statement with its Bid,the Bidder may revise and resubmit <br /> the Bidder's Qualifications Statement. <br /> 2. Other Documentation: Any other documentation required under the Mandatory or <br /> Supplemental Bidder Responsibility Criteria as set forth above. <br /> 3. Certification of Compliance with Wage Payment Statutes. To be filled in and signed by the <br /> Bidder <br /> The City may also require Bidders other than the apparent low bidder(s)to submit the above items. <br /> C. The following forms are to be executed after the award of Contract: <br /> 111 <br /> 1.
