City of Everett 01 3529 - 2 ' <br /> 2019 Biosolids Removal UT3705-2 <br /> A. The Contractor shall develop a Site Specific Health and Safety Plan addressing health <br /> and safety management methods specific to the project. The Plan shall, at a minimum, <br /> include: <br /> 1. The name of the individual at the jobsite responsible for implementation and <br /> compliance with this Plan. <br /> 2. If work requires the use of Fall Restraint or Fall Arrest, a Fall Protection work plan <br /> meeting the requirements set forth in WAC 296-155-24505 shall be included. <br /> 3. If applicable, the Plan shall include the name and qualifications of any electrical <br /> safety observer to be provided by the Contractor. <br /> 4. Steps taken to provide Protection of the Public and City Employees. This includes <br /> protection from falling objects. Indicated type of overhead safe guards and <br /> protection to be installed and safeguards from equipment damage due to debris <br /> (including nails) on the ground. ' <br /> 5. A description of tasks to be undertaken, and equipment mobilized for this project. <br /> 6. A list of all known and expected safety or health hazards, problems, and proposed <br /> control mechanisms. <br /> 7. Safety Data Sheets (SDS) of and procedures for using, disposing of, or storing for <br /> all chemicals, products, or materials regulated by WAC 296-62 to be used by the <br /> Contractor. <br /> 8. A list of personal protective equipment, monitoring devices, and hazard-specific <br /> plans or permits as appropriate and required by State and Federal regulations. <br /> 9. A description of emergency response measures, equipment available for <br /> emergency response to address accidents and releases of materials, including, <br /> but not limited to, first aid, eye wash/showers, and fire extinguishing equipment, <br /> and location of this equipment at the jobsite. <br /> 10. Emergency phone numbers contacts, and location of the nearest medical facility. <br /> 11. A monitoring and inspection plan and record keeping measures to ensure that <br /> equipment and work practices comply with this Plan. <br /> 12. Personnel names, training and notification procedures as appropriate to ensure <br /> that all jobsite personnel are familiar with the Plan elements. Include copies of <br /> training certificates. <br /> 13. Procedures for safe storage and handling of flammable liquids, in accordance with <br /> WAC 296-24-330. <br /> 14. If applicable the Contractor shall include procedures for safe storage and handling <br /> of compressed gasses in accordance with WAC 296-24-295, Compressed Gas <br /> General Requirement. <br /> 15. Other issues which the Contractor determines are appropriate and necessary to <br /> protect worker safety and health. <br /> 16. This plan shall be posted and marked in a location on site that will be noticed upon <br /> entry of work site. A fall protection plan must also be established and shall be <br /> posted next to the Health and Safety Plan (Accident Prevention Program). These <br /> postings are required to be protected from inclement weather and must be legible <br /> during the extent of this contract. <br /> 1.6 HEALTH AND SAFETY REPRESENTATIVE <br /> A. The Contractor shall designate a Health and Safety Representative who will oversee all <br /> work taken place on the job site including work performed by subcontractors and <br /> suppliers. The Health and Safety Rep should be capable of identifying hazards within <br /> 01 3529 - 2 HEALTH AND SAFETY <br />
