B. Vera P.C. Clinical Services. Vera P.C. shall provide certain Services at the Clinic location. <br /> These include without limitation: <br /> (1) Primary care,preventive care, health risk assessments to adult Participants,and non- <br /> emergency convenient care to pediatric Participants ages 3 and over,who are the dependents of <br /> Participants. <br /> (2) Electronic medical records; <br /> (3) Pharmaceutical assessment and management; <br /> (4) Health coaching; <br /> (5) Tailored health action plans; <br /> (6) Participant education; <br /> (7) Disease management and care coordination; <br /> (8) Quarterly analysis of claims data to identify gaps in care,patterns of disease and trends <br /> in Participant adherence to plans of care. <br /> (9) Ensure compliance with Medicare, Medicaid, state, federal, and other appropriate <br /> and relevant rules and regulations. <br /> (10) On-Site Laboratory Tests. <br /> (11) Specialist care coordination. <br /> (12) On-Site provider dispensed medications for routine one-time dose medications <br /> and starter packs for common maintenance medications <br /> (13) 24 hour nurse line <br /> (14) Optional Telehealth visits via Vera's sub-contracted service and passed through at <br /> cost based on actual usage. <br /> (15) Occupational health services as mutually agreed by the Parties. The specific <br /> occupational health services to be offered will be contemplated by the Parties subsequent <br /> to the initial launch of the clinic,workflows and administrative procedures <br /> collaboratively designed and trained, and then implemented at the clinic at a time <br /> mutually agreed post launch, and generally at least 6 months post launch.
