• On the job injury treatment(workers compensation visits) <br /> • DOT physical <br /> • DOT& Safety Sensitive drug screens <br /> • Norl-DOT pre-employment physicals and drug screen <br /> • Return to work exam <br /> • Fitness for duty exam <br /> Note: Services offered to non- Participants will be performed at Vera's then standard rates for <br /> the Services provided which is in addition to the Fees contemplated herein. <br /> Note: Prenatal care is not ffered and will be referred out. <br /> C. Additional Servic s. During the Term,Employer,after consultation with and the agreement <br /> of Vera and Vera PC, may decide to add to the list of Services and/or to increase Clinic capacity. If <br /> the additional Services or c pacity increase will not require Vera P.C.to change Clinic staffing levels <br /> or Participant experience a d/or clinic hours,and will not require any other change that may modify <br /> the cost to Employer or era or Vera PC of providing the Services, then, subject to a written <br /> amendment to this Agree ent and allowing for reasonable time for implementation and training by <br /> Vera and Vera PC, the ad itional Services and/or capacity shall be added to the Services. If the <br /> additional Services or increased capacity requires Vera or Vera PC, in their reasonable discretion, to <br /> change Clinic staffing levels or Participant experience and/or clinic hours, or any other change that <br /> may modify the cost to Employer or Vera or Vera PC of providing the Services,the following process <br /> will be followed: (i) Vera (on behalf of itself and Vera PC, as appropriate) shall make a written <br /> proposal to Employer as to the additional service and the changes in the Fees hereunder (and if the <br /> proposal includes an increase in the Admin PPPM Fee,the proposal will provide detail for how that <br /> increase was calculated); (ii) Employer's HR Director shall review and approve or reject Vera's <br /> proposal in writing within 60 days of proposal submission; and (iii) the parties shall prepare and <br /> execute a contract amendment within 90 days of proposal approval. If Employer does not approve <br /> the proposal within such 60 day period, the proposal will be deemed to be rejected. Amendments <br /> under this Section C,except for changes to the Admin PPPM Fee,may be executed by the Mayor on <br /> behalf of Employer without Everett City Council approval. <br /> D. Clinic Hours and losures: Vera and Vera PC shall work with Employer to develop the <br /> Clinic hours that facilitate t e greatest engagement potential for the Participant population. Based on <br /> the staff shown in Section below,the Clinic will be open 45 hours/week with at least one M.D. or <br /> ARNP on duty,unless the mployer's HR Director agrees in writing to a Vera proposal for different <br /> hours. These hours will b mutually developed in the context of engagement creating a reasonable <br /> work schedule for Vera P. . Clinic staff,and providing for: <br /> • at a minimum 9 da s for holiday closures,2 to 4 days for Vera company all staff meetings, <br /> and with reasonable notice to Employer,closures from time to time for necessary Vera P.C. <br /> clinic staff training. <br /> • In addition,provider visits may be limited from time to time to allow for planned vacation <br /> and unplanned sick or other time off. Employer has the option to request a locums <br /> Schedule 1, Page 4 <br />
